Instructions: How to Return the Home Sleep Test Kit
- Please take great care when dismantling the kit, keeping all the various parts together.
- Please place all parts in the black carry bag
- Place black bag into the travel box.
Return using the Royal Mail Online Service
Access the following link https://send.royalmail.com/
- Select United Kingdom 1 Kg Weight.
- Select Small Parcel.
- Important: Select Value of ‘£1000’, which should automatically select ‘Royal Mail Special Delivery Guaranteed by 1pm’ as the postage choice.
- Destination: Use postcode S17 3QR and house number 33.
- Fill in your own Address and Contact Details (which they need in case of a failed delivery).
- You may be able to select ‘Parcel Collect’ from your home/ office. If you wish to do this, any extra fee will also be reimbursed.
- Please pay the Royal Mail Fees.
- Simply pass on the receipt / invoice to us via email (info@breathemedical.co.uk), to be fully reimbursed back to your account.
